Abstract

This study was done to assess the outcome of Ilizarov method in treatment of infected femoral nonunion.
The pathogenesis of post traumatic osteomyelitis, the biomechanics of Ilizarov, the effect of weight bearing during Ilizarov treatment and the Ilizarov method in treatment of infected femoral nonunion were discussed.
Likewise the study reviewed the methods of assessment of bone and functional results according to ASAMI protocol. Furthermore the study briefly outlined the treatment options for such condition.
Detailed preoperative planning was discussed including the physical exam; investigations as well as the detailed surgical technique were described.
Union was achieved in all except two patients. Infection was eradicated in all except one patient.
Complications encountered in our study. However few patients were not satisfied due to joint stiffness, limping and other complications. PTI was the most common complications.
Despite these complications, the main goal of surgery was achieved with respect to union90% and eradication of infection95%.
The study results were discussed in comparison with the published literature.
In conclusion, The Ilizarov technique is a good salvage operation for infected nonunion of the femur. Limb salvage is preferable to prosthesis if the limb is viable, adequately innervated and the patient is mentally and financially committed to save the limb.
Conclusion
• Infected nonunion of fractures is one of the major problems in orthopedic surgery.
• There are many alternatives in the management of infected nonunion.
• Pre operative work up is the key for successful management through careful history taking, physical examination and investigation.
• Conventional methods of nonunion treatment are successful in cases of noninfected nonunion.
• Two phases of treatment are necessary to heal an infected draining nonunion: healing of the bone and elimination of the infection. These may be done sequentially or concomitantly to save valuable time.
• The Ilizarov technique is an important treatment method for surgeons performing posttraumatic reconstructive surgery. Non-union, infection, shortening and deformity are all addressed simultaneously.
• Although Ilizarov is commonly used for the treatment of infected femoral nonunion, it is associated with a high rate of complications.
• Despite high rate of complications; in experienced hands; union and eradication of infection could be achieved in most cases.
