Abstract

Poliomyelitis is an acute viral infection with severity ranging from inapparent infection to paralytic disease of the flaccid type, most commonly affecting lower extremities. Poliomyelitis is still on important public health problem In developing countries , including Egypt although it has been regionally eradicated from many countries in the world. The control of polio has been given a special priority by WHO through inclusion in its Expanded Programme on Immunization .- !EP 1) The aim of the study was to determine the reported i nc i dence of acute paralytic poliomyelitis In Egypt during the period !980-1991 to study the trend and seasonal variation of polio to study the effect of the expanded programme on immunization on the Incidence of’ poliomyelitis over­ tIme and to ~tUdV the ~ldeml01ogical characteristics of polIomyelitis. The field work i nel uded t.hree mai n parts . 1­ collectIon of’ all available data pertaining to the period from January 1980 to December­ 1991 for the-all acut.e paralytic poliomyelitis cases In Egypt to repoted to the ministry of Health ( MOH ) mainly -From the Institute of Poliomyelitis and rehabilitation( IPR ) and some sentinel units.2­ Collection of data pertaining ’to the epidemiological and clinical information of clinically diagnosed acute paralytic poliomyelitis cases which occurred in IPR.: during the This was collected period from Januarv to December 1991 using a speciaJly designed questionnaire interview. ­ Collection of data pertaining to the period from January 1980 to December 1991 ~or all acute paralytic poliomyelitis cases to repoted to the Alexandria Sentinel Unit In EI-Shatby Pediatrlc University Hospital The study revealed the following main results :­ 1­ The highest incidence rate in Egypt during the period from 1974-1991 was 1n 1974 ( 55.01/ 100.000 estimated population at risk) 2- A decline in the incidence rate durino the period 1980-1991 was observed in each of the following governorates : Cairo Qualyoubia t1onou~ia . Giza , sentinel unit of Alexandria (covering Alexandria Beheira and Kafr-El-Sheikh governorates) and in the other t-emC!.i ni nq governorates. in Egypt. ­ 3-The highest incidence rate during the specified period in both cairo and Qualyoubia occurred in 1980 while in Alexandria SentInel Unit . Monou~ia and Giza it was in 1982 . 4­ The highest trend value was in 1975 (40.98/100.000 estimated population at risk) while the lowest trend value was in 1989 (6.79/100,000 estimated population at risk) 5-Poliomyelitis cases are prevalent all the year round with tendency to Increase In summer and early ~a]l 6­ Most of the admitted cases to the IPR 86~were from only ~our governorates namely : Cairo, Giza Qualvoubia and Monoufia . 7­ One of the main -features o-f the disease was its predominantly in~antile character 81,2 of cases occurred dur i ng the first two years of life . 8­ The disease had a male prepondrance with a male to -female ratio o-f 3:;2 . 9­ About two thirds of cases {59.?!:) were -from rural areas.
