Abstract

Total hip replacement is the most common reconsu ctive procedure currently performed all over the world.
Continued evaluation of the use of cemented tot’1! hip prosthesis has revealed that mechanical failure caused by aseptic loosening of the components has been the most significant and frequent long-term problem, specially in high risk, youn:.^ and active patients.
These findings have stimulated the research for another method of fixation by biological ingrowth that is cementless totalhip replacement for such high risk young patients.
The goal of cementless total hip is to achieve proximal fixation by bony ingrowth and to avoid distal stress transfer.
The aim of this work is to assess the results of different types of cementless total hip arthroplasty in the treatment of complicated femoral neck fractures in young adults such as, highly comminuted fractures, neglected cases, failed internal fixation or cases complicated with non union, avascular necrosis, malumon and osteoarthritis.
Our study were done on 35 young adult patients (36 hips using designs of the cementless total hip prosthesis, shells were either titanium HA coated or Co-Cr porous coated or porous and HA coated. The inserts were ceramic or polyethylene.
