Abstract

This study was carried out in Shebin El-Kom city and its district.
AnINtervention-training workshop on communication skills was done for health team in the rural and urban areas chosen by simple random sample and assessments for their knowledge, attitude and practice were done on 3 stages of the study (before, immediately after and 6 months later after the training).
Also 300 patients each group (100 patients in each stage of the study; 50 patients from the urban area and 50 from the rural area) were assessed for their satisfaction and compliance.
The aim of this work was trying to improve communication skills of the primary health care team through in-service conducted training course and to study the impact of primary health care team communication skills training on knowledge, attitude and practice of the health team and on their patients’ satisfaction and compliance.
The fieldwork started at June 2003 and ended in May 2004.
The studied health team were subjected to a questionnaire on their personal data, knowledge and attitude towards communication skills also their practice was assessed by a checklist.
Also the 3 groups of patients were subjected to a questionnaire on their socio- demographic characteristics, satisfaction and compliance.
Data were analyzed using descriptive statistics (mean and +SD), Wilcoxon signed rank test, Mann – Whitney U test, Kruskal Wallis test and Pearson correlation The mean scores for knowledge, attitude and actice of health team and for patients’ satisfaction were significantly higher in the rural area before the training.
There was significant increase in the knowledge and practice in communication skills in the general practitioners and specialized physicians consequently before the training.
Immediately after the training the mean score for knowledge and attitude of health team had significantly improved and this improvement was sustained 6 months after the training except for knowledge, which had significantly decreased.
lso, significant improvement in practice of health team occurred immediately after the training except for eliciting patient expectations, summarizations and clarification of details, which did not improve. The improvement in the health team practice significantly decreased 6 months after the training except for greeting, use of verbal communication techniques and Non-verbal communication techniques, which were sustained.
Also, significant improvement in all the items of patient satisfaction has occurred except for satisfaction to time factor and the resultant improvement was sustained 6 months after the training except for listening without interruption and giving health education about the disease items, which had significantly decreased.
Patient compliance to medication did not significantly improve after the training but compliance to behavioral, follow up and overall compliance had significantly improved and this improvement was sustained except for compliance to behavioral recommendation, which had significantly decreased 6 months after the training.
