Abstract

Affective syndrome associated with cerebrovascular diseases
represent an important area for research, although many of the clinical
characteristics have been reported in the literatures for years, still more
research and studies should be undertaken, and several studies have now
demonstrated that post stroke depression is a frequent sequel of
cerebrovascular diseases with incidence about 30 - 50%, and that the
anterior lesions, particularly those involving the left frontal cortex and
left basal ganglia are strongly associated with major depression.
The aim of this work was to determine the prevalence of
depression after cerebrovascular accidents, its possible different
etiological factors include age, sex, marital status, educational level,
history of smoking or other drug or alcohol abuse, premorbid
personality, past psychiatric history, family history of psychiatric
disorder, lesion location, degree of weakness with the type of depression.
And that in both Egyptian sample of patients as well as a Saudi
sample of patients.

For this study each patient was subjected to:-
1- Full neurological assessment.
2- Full psychiatric sheet and examination using the DMS III R
criteria for diagnosis of the psychiatric illness
3- Brain CT Sean to confirm the neurological diagnosis and to
detect the site of the lesion.
4- Beck Depression Inventory Scale (BDI) and Hamilton Rating
Scale for Depression (HAMD) to measure the degree of
depression.
5- Social and Occupational ’”Function Examination Scale
(SOFAS): to assess the social and occupational function.
6- Mini Mental State Examination: to assess the cognitive and
intellectual impairment.
Our study showed that the total prevalence of depressive illness in
the experimental group is 40%, 36% in the Egyptian group and 44% in
the Saudi group.
No significant differences showed between the Egyptian group
and Saudi group.
Also in the total of the experimental group, there was a significant
correlation between major depression after cerebrovascular stroke and
the degree of physical disability, lesion location, the progressive course

State Examination, introvert premorbid personality and a positive family
history of psychiatric disorders, and a personal past history of psychiatric
illness
There were no significant correlation with other parameters.
The time interval between the incidence of stroke and appearance
of a depressive disorder (in months) was 2.12 for minor depression and
0.906 for major depression.
We concluded that post stroke depression may be a multifactorial
in etiology (lesion location, severity of physical disability, intellectual
impairment, or genetic loading ofpsycbiatric illness). We recommended
that psychiatric examination is an important and essential part in the
examination of stroke patients for psychiatric sequale of stroke and
starting the suitable rehabilitation programs. Early diagnosis of post
stroke depression is very important for the rapid recovery and good
prognosis of such patients.
