Abstract

Breast conservation surgery is developing to be surgery of choice in treatment of certain breast cancers aiming to remove the tumor yet maintaining the symbol of feminity.
Between being radically mutilating and dangerously conserving comes the balance, the plastic and general surgeon should consider when offering the patient the treatment to her breast cancer.
The role of post operative radiotherapy and chemotherapy is increasingly proved in preventing the tumor recurrence ,yet the tissue damage and effect on the breast reconstruction options is increasingly understood .
Tumor recurrence whether a true recurrence or originally missed tumor cells can be related to many factors of personal, surgical and tumor factors.
After breast conservation, aesthetic evaluation of breast varies from patient to another based on tumor, patient and surgery factors. It could be assessed using objective and subjective methods. Thus what may be aesthetically pleasant to patient may be unfavorable to the plastic surgeon.
Breast defect can be replaced during the surgery of breast conservation by what so called oncoplastic procedures that gives the surgeon the chance of being more radical in tumor resection hence increasing the oncological safety yet maintaining the breast shape to great extent.This can be achieved via incorporating the tumor in plastic procedure such as breast reduction and breast mastopexy.
Other options of volume replacement are local flaps from the remaining breast tissue as in rhomboid flap, rotational flap and advancement flap.
When breast size is small thus local flaps are not of choice. Regional flaps from nearby axilla or the inframammary region can offer a good results.
