Abstract The aim of this work is to study the possible role n prpotaglandine in the pathogenesis of IBS and to assess the therapeutic value of administrating prostaglandin-synth-esis inhibitors ip ameliorating the IBS symptoms. This study was carried out op 20 IBS patients, (10 patients with predominant diarrhtand 10 patients with predominant constipation)las well as 10 normal subjects who were taken as control group. All the patient and control groups were subjected to the following investigations and proved to be normal. 1-Full history and thorough clinical examination. 2-Complete urine and stool analysis. 3-Complete blood picture. 4-Colonoscopy. r> Then ’ 5- Serum PGE levels were estimated. 6- The patient groups were treated with indomethacin (75 mg) twice daily for 7 days, and serum PGE was re-esti-mated after treatment. Our result demonstrated the following data:-( 1- A very highly significant increase of serum PGE level in IBS patients with pre-dominant diarrhea compared to control group. 2-A very highly significant decrease of serum PGE level in this group of patients after indomethacin treatment compared to those before treatment. 3-A significant decrease of serum PGE level in IBS patients after indomethacin treatment compared to control group. 4-Clinical improvement of the diarrhea in this group of patients after indomethacin therapy. Insignificant increase of serum PGE level in IBS patients with predominant constipation compared to control group. 6-A significant decrease of serum PGE level in this group of patients after indomethacin tratment compared to those before treatment and the control group. 7-No clinical improvement of constipation and abdominal , pain in this group of patients after indomethacin trea- tment despite the significant decrease of serum PGE level. Thus we came to the conclusion that PGs my paly an important role in the pathogenesis of diarrhea in cases of IBS, and indomethacin as prostaglandin synthesis inhibitor may be effective in ameliorating this symptom. However PGs may have no role in the pathogenesis of constipation and abdominal pain in IB.S and indome-thacin is not effective in ameliorating these symptoms, and other :actors may be important. |
