Abstract

All patients was subjected to:
1- Full and detailed history.
2- Complete clinical examination:.
3- Laboratory investigations:
-Renal function tests (serum creatinine, blood urea)
- Liver function tests including: aspartate transaminase, alanine
Transaminase, alkaline phosphatase, total proteins, serum albumin, serum
Bilirubin .
-Complete blood picture including platelet count
4 - Abdominal ultrasonography and measerment of spleen bipolar
diameter.
5 - Calculation of the platelet count/bipolar spleen diameter ratio.
6 - Upper gastro-intestinal Endoscopy
The results were statistically analysed and tabulated and reveald that the
patients were 56 (56%) males and 44 (44%) females; their ages ranged from
38-80 years old with a mean value of 54.95 years. They were classified into
two groups according to the presence or absence of oesophageal varices.
The platelet count/ bipolar spleen diameter ratio cut off with the best
sensitivity and for prediction of oesophageal varices( O.V) was determined
and was (898).

Summary and conclusion
The use of platelet count/ bipolar spleen diameter ratio in prediction
of O.V would be cost effective,simple and applicable.

Summary and conclusion
Conclusion
from the present study we can conclude that:
o Platelet count /bipolar spleen diameter ratio (898) is a simple,
accurate, sensitive and cost effective non invasive parameter for
prediction of oesophageal varices in cirrhotic patients
regardless the grade of cirrhosis.
o There is no relation between Child grades and the presence of
oesophageal varices

Summary and conclusion
Recommendation
from the presnt study we can recommend that:
 Patients with liver cirrhosis should be assessed when first seen
for the presence of oesophageal varices.
 Regular use of platelet count / bipolar spleen diameter ratio as a
cost effective non invasive parameter for prediction of
oesophageal varices in cirrhotic patients.(cut off > 898 is a good
negative predictive test)
 Upper gastrointestinal endoscopy should be limited for cirrhotic
patients probably have oesophageal varices according to results
of non invasive parameters ( platelet count /bipolar spleen
diameter ratio < 898).
