Abstract

The primary objective of reperfusion is not only restoration of blood flow in the epicardial coronary artery but also complete and sustained reperfusion of the myocardium.
Therefore we studied the effect of different doses of intracoronary nitroglycerin on myocardial blush in patients undergone successful PCI. Our study included 30 patients with CAD who undergone successful PCI, all patients were given 200µg of IC NTG in two divided doses after PCI. TMPG was assessed in all patients before PCI, after PCI, after first dose and after second dose of IC NTG. There was a high significant improvement in TMPG after PCI . We found that 56% of our patients who had TIMI 3 after PCI had TMPG 0 or 1 so that TIMI 3 does not necessarily indicate myocardial salvage. Moreover, there was a high significant improvement in TMPG after IC NTG specially after second dose of IC NTG by marked increase in number of patients who achieved TMPG 3 (50% of patients). Also Corrected TIMI Frame Count (CTFC) showed improvement after PCI and more improvement after IC NTG by decrease in cine frame count after PCI and after second dose of IC NTG.
