Abstract

This study included 45 patients, with acute respiratory failure due to exacerbation of COPD divided into three groups each of them 15 patients. The first group was represented by patients who received standard therapy alone, while the second group included patients who received standard therapy plus CPAP. Third group included patients who received standard therapy plus BiPAP. The need for intubation was decreased in BiPAP group (2) patients 13.3% than CPAP group (5) patients 33.3% and standard therapy group (7) patients 46.7%. hypoxemia was marked in those who was on BiPAP support and CPAP it was mostly related to mask leak. It can be concluded that, CPAP and BiPAP as form of NIPPV can reduce in intubation rate in COPD exacerbation but without reducing the PaCO2 to the basal level which is the primary target on dealing with such case. BiPAP decrease incidence of hospital stay more than CPAP, with less complications. NIPPV used in treatment of COPD patients with acute exacerbations and RF decreased durations of ICU stay and so decreased cost for treatment of this patients
