Abstract

The present work entails the evaluation of operative technique of Anterior Seromyotomy with Posterior Truncal Vagotomy in the management of 15 patients with chronic duodenal ulcer who are candidates for surgical treatment.
The operation aims at denervating the fundus and corpus of the stomach by cutting the posterior vagal trunk in the abdomen and cutting the branches of the anterior nerve of latarjet beneath the serosa along the anterior aspect of the lesser curvature of the stomach. The vagal nerve branches of the anterior nerve of latarjet (Crow’s foot) to the antrurn and pylorus are left intact, thus preserving the antral !!! function and the antroduodenal pump mechanism, and hence avoiding the need for a drainage procedure with its drawbacks.
The patients have been followed up for 2 years after surgery. The success of the operation has been evaluated both subjectively and objectively-using endoscopic examination and utilizing the Uisik grading.
In 93% of patients the dyspepsia has disappeared and endoscopic healing of ulceration has been confirmed.
The Uisik grading has been found excellent in 53% and good in 27% of the patients.
