Abstract

Introduction :- Acute myocardial infarction (AMI) is the world’s leading cause of mortality and morbidity . Therefore, quick and reliable diagnosis of AMI is extremely critical. Objective – To evaluate the more sensitive and specific marker(s) in diagnosis of AMI to reduce the damage occurs. Patints and Methods :- The study included 43 patients besides 20 healthy person to serve as controls . Venous blood samples were taken on admission (t=0) , 6h (t=6) and at 12h (t=12). The serum samples were assayed for T-Ck, CK-MB activity, CM-MB mass , CTn-I , CTn-T , IL-6 , hsCRP , T-LD and its isoenzymatic activities by electrophoresis and Echocardiogram (Echo) pattern was performed for all cases at admission only. Results :- All studied markers showed specificity 100 % in diagnosis of AMI except IL-6 has specificity 80 % . CTn-I has highest sensitivity for detecting minor injury 95.5% at zero time , 100 at 6,12 hours intervals. Total LD has lowest sensitivity 45.5 % at zero time and (LD1+LD2) isoenzymatic activities separated by electrophoresis (LD1+LD2) has sensitivity of 93.2%at zero time with specificity 85% . T-CK , CK-MB activity , CK-MB mass , hsCRP has a good correlation with Echo data ( LVED, EF, FS, Score Index) P=0.00 at zero time , 6 and 12 hours. Conclusion :- CTn-I was more sensitive and specific marker for the early diagnosis of AMI while CK-MB activity , CK-MB mass and hsCRP has a good correlation with Echo data ( Score Index ) which represent the infarct size.
