Abstract

The most common cause of death among burn population nowadays is inhalation injury. Using of fiberoptic ‎bronchoscopy and bronchoalveolar lavage for diagnosis and treatment of inhalation injury help greatly in early ‎diagnosis and proper management of inhalation burn. ‎ Material & Method: Patients with cutaneous burn and clinical manifestation suggesting inhalation injury. ‎Bronchoscopy was done to confirm the diagnosis. Patients classified into two groups the bronchoscopic ‎group: for which repeated therapeutic bronchoscope and bronchoalveolar lavage was done. The ‎conventional group: using medical treatment only. Cytological examination was done for bronchoalveolar ‎lavage fluid.‎ Results: bronchoscope and bronchoalveolar lavage is more effective in reducing complications, length of ‎hospital stay and mortality of inhalation burn patients than conventional methods of treatment. Also more ‎rapid improvement was noticed clinically and laboratory in bronchoscopic group.‎ Conclusion :‎ ‎1.‎ Using a combination of early bronchoscopy together with clinical examination and A.B.G measurement ‎to diagnose and evaluate the accurate severity of the inhalation injury is a must.‎ ‎2.‎ Use of bronchoscopic regimen with bronchoalveolar lavage using heparin had bitter results without ‎adding any further risk more than the conventional treatment.‎ ‎3.‎ B.A.L. cytology performed to patients has good diagnostic and prognostic values to estimate the ‎progression and resolution of the inflammatory response
