Abstract

Diagnosis of pelvic swellings may be a challenging problem of the gynecologist. The diagnostic accuracy of pelvic examination varied in different series between 50% and 90%. It is affected by several variables including the experience of the gynecologist, the type of patients and the characteristics of the swelling. Several factors may hinder the diagnosis, even for an experienced gynecologist; varginity, obesity, associated pregnancy, ascites and pelvic tenderness. Many diagnostic procedures have been used for a thorough evaluation of such cases. Among them, transvaginal ultrasonography and Magnetic resonance imaging are currently increasing in popularity. Magnetic resonance imaging has demonstrated potentiality in evaluation of adnexal pathologic conditions and has been used in diagnosis of various types of adnexal masses. In some studies MRI has been used retrospectively compared with transabdominal ultrasound. While in others both transabdominal and endovaginal ultrasound were compared with MRI in evaluation of pelvic masses. In these studies, ultrasound from either approach was as sensitive as MRI in the characterization of adnexal masses and its differentiation as simple, solid or complex. The purpose of this prospective study was to further evaluate the comparative accuracy and utility of MRI and TVS in the diagnosis of adnexal masses.
