Abstract

The study was conducted in Cardiothoracic Surgery Department, Mansoura University Hospital from March 1997 to March 2002. The study included 50 patients who had combined aortic and mitral valve disease and who underwent combined aortic and mitral valve replacement using bileaflet mechanical prosthesis. The rheumatic fever was the commonest cause for the combined aortic and mitral valve disease (42%) and also, considered main cause for each group except group III where, chronic non specific valvulitis was the commonest. This study showed that combined aortic and mitral valve replacement is still considered as a high risk morbidity and mortality surgery owing to a group of risk factors. On the light of this study, we can conclude that aortic and mitral valve replacement surgery is well assessed regarding the following: The variety of the disease combination,Left ventricular performance,NYHA functional class for each patient,Atrial fibrillation association,Mean pulmonary artery pressure,The ischemic time during the surgery,The preservation of the mitral valve apparatus to maintain left ventricular geometry and prevent deterioration of the left ventricular systolic function.
