Abstract

Aim of work: The aim of this study was to detect the value of MRCP in detection of level and causes of extra-hepatic biliary obstruction. Patients and methods: The study included 48 patients (25 males & 23 females) with mean age 51 years who suffering from obstructive jaundice After full clinical and US examination the patients were examined with a 1.5 tesla MR unite using MRCP HASTE sequences (both single slice acquisition & Multislice acquisition followed by MIP reconstruction). 33 patients then was referred to ERCP after 1-2 day and 8 patients underwent PTD. Then results of MRCP was compared with ERCP results in cases who underwent both modalities. Results: MRCP achieved high success rate reach 98% compared to 92% for ERCP. It revealed high sensitivity reaches 96% and specificity of 100% in detection of the cause & level of obstruction in 31 patients compared to sensitivity & specificity 100% of ERCP in diagnosis of these cases. Conclusion: MRCP considered safe, fast, non invasive, accurate procedure in detection of cause and level of in the extra-hepatic biliary obstruction. It should limit the role of ERCP in diagnosis of extra hepatic biliary obstruction to cases which need therapeutic interference in the same set of diagnosis.
