Abstract
The aim of our study was to evaluate circulating adhesion molecules in diabetic patients, and to evaluate VCAM-l and E-selectin as markers of endothelial dysfunction in diabetic nephropathy and retinopathy, and the effect of glycemic control. Our study comprised 45 patients with diabetes mellitus( 21 patients with type 1 and 24with type 2 ) 21 were males, and 24 were females.Their age ranged from 13 to 70 years with average age of 45 7( 15 patients without microvascular complication as group l,and 30 with retinopathy and/or nephropathy as group 2) and 10 heathy subjects of matched age and sex as control group. All patients were subjected to thorough history taking and clinical examination with stress on weight- hight, blood pressure, symptoms suggestive of microangiopathic complication, non of them had history (or evidence) of anaemia, hepatic disease or overt renal insufficiency. Patients with clinically symptomatic macro angiopathy were excluded., those with pyuria, haematurea were excluded. 45 patients were treated with anti-diabetic oral sulphonylureas alone, in combination with metformin or with insulin alone, the following investigations were done at the beginning of the study :-ECG, fundus examination, urinary albumin in 24h, plasma lipid profile, fasting, 2 h. post-prandial blood glucose, HbA1C, VCAM-1, and E-selectin, and revaluated after 6 months of glycemic control.
