Abstract

Renal transplantation had become the treatment of choice in the managemnt of chronic renal failure . Many serious complications can cause renal failure in the transplanted kidney. In this study we tried to compare the diagnostic value of power Doppler sonography with that of color Doppler resistive index and spectral wave form in the detection of early post kidney transplant complications. Our thesis included 55 cases of renal transplant recipients (44 males and 11 females) with mean age of 34 years. Duplex Doppler and power Doppler sonography were done for all cases daily from the first day post transplantation. According to that we considered the first day scanning to be our control group except-for one case in which graft dysfanction started at the first day, so our control group were 54 cases. US guided biopsy was done in 33 cases. The complications which had been encountered during our study include: mild acute rejection grade I (15 cases), moderate acute rejection with vasculitis grade II (4 cases), marked acute rejection GIll (one case), border line rejection (3 cases) acute tubular necrosis (4 cases), cyclosporine toxicity (6 cases) graft artery stenosis (one case), graft artery thrombosis (2 cases), segmental infarctions (2 cases), arterio¬venous fistula (one case) , obstructed graft ( one case) and (4 cases) were diagnosed after treatment. We found that conventional and power Doppler sonography had no role in diagnosing cases of border line rejection and cyclosporine toxicity as there Doppler findings were normal. While in other different grades of acute rejection Gl, GIl and GIll we found that power Doppler ultrasonography had high sensitivity and specificity 82% and 100% respectivetly and positive predictive value of 100% and negative predictive value of 71 % in diagnosis of acute medical dysfunction esspcially acute rejection were cortical hypoperfusion was a constant sign in all cases except one case with normal CVP and pathologically proved
