Abstract

In the present study, comparison between common treatment modalities in primary open angle glaucoma including (Timolol 0.5% = Cusimoiol eye drops, Dorzolamide 2% = Trusopt eye drops, Latanoprost 0.005% = Xalatan eye drops & Subscleral trabeculectomy = S.S.T.) was performed, as regards their effect on the following parameters: 1) Magnitude oflowering lOP. 2) Progress of visual field changes. 3) Progress in the changes ofRNFL thickness. The study was conducted on 200 eyes of 100 subjects who were both age sex-matched & they were categorized into 5 main groups (20 subjects in each group = 40 eyes). These groups were: . Group I: Included patients with POAG recervmg topical r3-blocker eye drops (Timolol maleate = Cusimolol _. Cusi) twice daily. Their age ranged between (34-70 years) . . Group II: Included patients with POAG recervmg topical carbonic anhydrase inhibitor eye drops (Dorzolamide == Trusopt ,- MSD) three times daily. Their age ranged between (34-7 5 years) . .Group III: Included patients with POAG receiving topical prostaglandin eye drops (Latanoprost = Xalatan-Phannacia & Upjohn) once daily at 8.00 PM. Their age ranged between (30-70 years). Included patients with POAG subjected to subscleral trabeculed (S. S. T.) as the initial line of treatment and all the operations were perform,’ the same surgeon. Their age ranged between (34-65 years). There wa: Group V: Included normal subjects and considered as a control comparison. Their age ranged between (34-60 years). Progressit visual field in: All patients were subjected to the following: . 1- Thorough history taking (including both general and ocular history). 2- Measurement of the best corrected visual acuity. 3- Slit-lamp examination of the anterior segment. 4- Ocular tension measurement using Goldmann applanation tonometer. 5- Gonioscopy using Goldmann 3 mirror contact lens. 6- Fundus examination using Goldmann 3 mirror contact lens. 7- Refraction with Canon Auto. Ref. R.30 8- Perimetry using Humphery visual field analyzer (central 24-2 program);l 9- Coloured fundus photography & red-free RNFL photography. 10- Optical coherence tomography (OCT) for measurement thickness. Our results showed that, all lines of treatment of POAG cases used intl study resulted in a statistically highly significant lowering of the mean lOP tT, the base line value as compared to the control group (P = 0.000 I). 1- Mean dev 2- Corrected field cham There w ,y group III ] The highest percentage of pressure reduction was noticed among patie , m group IV (40.43 ± 6.09%) followed by group 1II (33.56:1: 5.01 %), then grd I (26.34 ± 4.02%) and finally the least percentage of pressure reduction found among patients in group II (23.45 ± 3.88%). Patients with POAG in groups (I -- IV) had lower initial RNFL thickn measured with OCT compared with the control group.
