Abstract

The complaint of back pain in children must be regarded as highly significant and every attempt should be made to determine its cause. Minimal clinical signs and apparently urelated symptoms may be the clue to major pathology. The etiological factors can be separated into 5 main categories: mechanical problems, developmental abnormalities, inflammatory processes, neoplastic processes and hysterical. Also, there is a miscellaneous group of factors, causing this syndrome. It is also helpful to classify back pain in children according to the age of the patient where there are some diseases affect children under the age of 10 and other diseases affect them more commonly above this age. Speaking about the clinical picture of back pain in children, we have to deal with the symptoms, signs, physical examination, laboratory data and x-ray evaluation. The differential diagnosis of back pain in children is dependable upon the discussion of every etiological factor as a single item. The child who has back pain in which a diagnosis cann’t be firmly established presents a difficult clinical dilemma. Frequently, a decision must be made to pursue the diagnosis with more extensive testing or to proceed with nonspecific treatment. If a careful history, detailed physical examination, and high-quality radiographs don’t lead to a specific diagnosis, consrevative treatment may be initiated, with the understanding that continued and careful periodic.
