Abstract

Slipping capital femoral epiphysis affects child¬ren in a fairly narrow range of age. The lesion only affects the upper femoral epiphysis. The etiblogy of slipping capital femoral epiphysis is not yet known. Key (1926, Harris (1950), Howorth (1957) and Trueta ” (1957), have suggested possible causes, each of which proposes some mechanism by which the epiphyseal plate is weakened. A shearing strain is present at the epiph¬yseal,.plate during walking and running, the forward mov¬ement of the femoral neck instance phase of gait being resisted by inertia of the body weight. Therefore the basic etiologic process, weakens the epiphyseal plate and makes it more vulnerable to this shearing strain. The diagnosis is usually not difficult to make. The average age incidence for girls is 11 - 13 years, and in boys is 13 - 16 years. Bilateral slipping in 10 - 40%, and the left hip is affected more th~ the right, and boys more than girls. - 160 - Pain in the hip, thigh or knee, and limping with external rotation of the limb are the main cardinal signs of slipping capital femoral epiphysis. Antero-posterior and frog-leg position of roento¬gram are more valuable in detecting early slipping. Watson Jones (1962), stated that the treatment of displacement of the upper femoral epiphysis is not a very happy chapter in the history of orthopaedic surgery. A Yariety of orthopaedic methods have been employed, and some with disastrous results, particularly when the blood supply to the femoral head has been given too little o”onsideration. The first step in adequate treatment is a high index of suspicion which increases the incidence of early diagnosis while the slip is minimal. The patient should be taken off weight bearing inuned’iately, and admitted to the hospital on an emergency basis and early management to prevent further slipping.
