Abstract

Snoring is a great medical and social problem which may be accompanied by obstructive sleep apnea syndrome (OSAS). It is common among obese persons and may be due to upper airway abnormalities. Uvulopalatopharyngoplasty(UPPP) is one of the most common surgical procedures designed for this syndrome. However, much controversy has existed regarding its indications and its effects on snoring, sleepiness, apnea/hypopnea index and on oxygen saturation. This surgery was designed to widen the upper airway at the level of the soft palate and tonsils, but it has no opportunity to treat a lower pharyngeal obstruction. The suitable candidates can be selected on the basis of the case history, clinical examination, cephalometry, nasopharyngoscopy and polysomnographic study. Recently, intra-luminal pressure analysis of the pharynx and esophagus during polysomnographic study has evolved to be a valuable method in preoperative assessment of the obstructive site and postoperative evaluation of the OSAS surgery. The present study was conducted on four groups of patients: the first group consisted of 34 Egyptian patients with obstructive sleep apnea, 21 males and 13 females, with age ranging fromlG to 62 years and body mass index (BMI) ranging from 24 to 41 Kg/m^. The diagnosis was based on the severity of snoring, the severity of sleepiness, the clinical assessment of the soft palate and tonsils, lateral cephalometric study, physician counting of the number.
