Abstract

Absorption of irrigating fluids during transurethral prostatectomy, is a dangerous and may be fatal complication, unless recognised and vi/ell managed early. This study i/as conducted on twenty male patients sub¬mitted for elective transurethral prostatectomy, classified into two equal groups . Epidural and spinal anaesthesia were used one for each group. Distilled water and glycine were used as irriga-nts in all patients and duration of use of each was recorded. Arterial blood pressure and heart rate were measured every fifteen minutes up to two hours and the results were compared with the basal values. There was a significant de¬crease in heart rate and increase in blood pressure due to fluid absorption and blood volume expansion. Heparinized blood samples were drawn for estimation of serum- sodium, potassium, haematocrit, blood and plasma haemoglobin after one, two, six, and twenty-four hours and the results were compared with the pre-operative values . Serum sodium and blood haemoglobin were significantly de¬creased after TURP due to dilutional effect mainly. Serum potassium was significantly elevated, this may be due to haemolysis, acidosis, coupled sodium-potassium diffusion or leakage from injured prostatic cells. Plasma ( free ).
