Abstract

This work was done to clarify the prevalence of Y. enterocolitica infections among chronic liver disease patients. This is besides, detection of pathogenicity and determination of susceptibility of isolated strains to different antibiotics. In this study, 615 chronic liver disease patients (319 males and 296 females) admitted and attending the Tropical Medicine, Internal Medicine Departments in the main Mansoura University Hospital (MUH) and Pediatric Gastroenterology Department in the Mansoura University Children Hospital (MUCH) were studied. Fifty personnels with manifestations of gastroenteritis and no underlying liver disease were selected as a control group. 615 stool samples were collected from cases diagnosed clinically as gastroenteritis. In addition to stool samples, other samples were collected from these patients according to the suspected other clinical sites of infection. This study revealed the following results: Although low prevalence rate of Y. enterocolitica among patients in our study yet the organism should be looked for and not be ignored as a cause of gastroenteritis. Yersinia enterocolitica infection in chronic liver disease patient may affect the liver function . Follow up study for patients with Y. enterocolitica infection is needed as the organism may produce acute or chronic liver diseases. The study of acute and chronic yersinosis requires a large population that is followed up for a sufficiently long period for development of chronic diseases. Attention should be paid towards Y. enterocolitica as possible agent for chronic liver disease. Cold enrichment of stool samples for isolation of Y. enterocolitica is not needed for prolonged periods. The most common Y.enterocolitica­associated infection is gastroenteritis. Yersinia enterocolitica­associated gastroenteritis is more prevalent in chronic liver disease patients of age group 30­39 years and in winter months. Yersinia enterocolitica is still showing sensitivity to wide range of antibiotics. Detection of virulence yst gene by PCR is more convenient for determination of virulence of the organism than plasmid detection.
