Abstract

Background: substance use disorder poses as a challenge not to be overlooked with clinical, prognostic and socio-economic impacts affecting patients with both schizophrenia and bipolar disorder. Studies show variation in occurrence of substance use disorder in the aforementioned disorders related to different clinical, environmental and socio-economic factors.
Aim of the Work: To assess and compare the occurrence of substance use disorder and its clinical history among patients diagnosed with schizophrenia and those diagnosed with bipolar disorder.
Patients and Methods: It was a cross-sectional, observational, comparative study conducted at Psychiatric inpatient wards at El-Mamoura Psychiatric Hospital.The sampling method was convenience sampling and the size of sample was 150 patients. For socio-demographic data, clinical data of either schizophrenia or bipolar and the history of substance use a predesigned semi-structured sheet was used. Structured Clinical Interview for DSM-IV-Clinician Version (SCID-CV) the Arabic version was used to confirm the diagnoses of both schizophrenia and bipolar disorder and to exclude any other psychiatric co-morbidity. To assess the severity of substance use disorder, Addiction severity index the Arabic version was the tool of choice.
Results: This study shows substance use disorder to be a real challenge to patients with schizophrenia and bipolar disorder, with a significantly high occurrence and a burden of a real deal affecting their clinical outcome and socioeconomic situation. Most patients were with a moderate to severe addiction severity score, and the severity would have been much worse if the study inclusion criteria had allowed patients with medical concerns to be within the subjects investigated. Occurrence of substance use disorder was higher in schizophrenia in comparison with bipolar disorder according to the results from this study.
Conclusion: This study shows how large the dilemma of substance use disorder in schizophrenia and bipolar disorder patients, with high occurrence, severity and a deleterious impact affecting clinical course, prognosis and socioeconomic level of the patient, and it doesn’t overlook the reciprocity between the such aspects and substance use disorder in schizophrenia and bipolar disorder patients.
