Abstract

Amputation itself is a change in body structure, but it has a great influence on many activities, participation in activities and quality of life. On the other hand, amputation causes a variety of physical and psychosocial challenges, including changes in body image and lifestyle, changes in self-concept, impaired physical functioning, use of the prosthesis, and phantom pain. Persons with lower limb amputation (PLLA experience reduced mobility, which leads to a number of difficulties ranging from self- care, independence and psychological well-being. Lower limb amputation (LLA) highly leads to physical disability because of the loss of a body part Lower limb amputation is a life-changing event and has an impact on functional, work, social and recreational activities (Alessa et al., 2022 The aim of this study was to determine the effectiveness of nursing intervention on phantom pain, activities of daily living and quality of life among patients with amputation. Settings: The study was conducted at the General Surgical Department and rehabilitation unit in Menoufia University Hospital in Shibin Elkom, Menoufia Governorate, Egypt Subjects: A consecutive sample of 60 adult patients with lower limb amputation who fulfilled the inclusion criteria was selected. A simple random sample was used by assigned patients divided into two equal groups of 30 patients for each (control& study). Study group: 30 patients who was received nursing intervention. Control group: 30 patients who was received only routine hospital care Instruments of the study: Four instruments were utilized for data collection: Structured interviewing questionnaire, Part 1: Socio-demographic Data Sheet Part 2: Medical History Sheet Modified Barthel Index (MBI World Health Organization of Quality-of-Life Brief (WHOQOL-Brief). Visual analogue scale (VAS) The main findings of the present study were: There was highly significant improvement in mean MBI score among study group than in control group at post, first and second follow-up- intervention (P < 0.001). There was highly significant improvement in mean of all domains of QOL (general health physical, psychological, environmental and social) among study group than in control group at first and second follow-up-intervention (P < 0.001). There was significant improvement in mean of visual analogue pain scale among study group than in control group at post, first and second follow-up-intervention (P 0 <005). Conclusion Based on the consequences of current study, designing and implementing a nursing intervention based on the needs of patients after lower limb amputation was effective in reducing phantom pain, improvement physical and psychological health, performance of activities of daily living (ADL) and quality of life (QOL). Implementing of nursing intervention strategy at the general surgical unit was effective when providing care as it encouraged patient to be an active participant in care and had a significant improvement of activities of daily living and quality of life among patients with amputation. The overall findings of this study provide evidence to support the value of nursing intervention for individuals with lower limb amputation. Recommendations Based on the findings of the current study, the following recommendations are derived and suggested: A-Recommendations for patients:  The nursing intervention should be considered as an integral part in the Hospital routine care of patients with LLA.  Continuous educational and training programs should be provided to patients with lower limb amputation on regular basis to improve their physical and psychological health and quality of life. A simplified, comprehensive and illustrated Arabic guided images booklet should be distributed for each newly admitted patient diagnosed with amputation.  Establishing a poster in surgical word, including all pertained information of lower limb amputation and all aspect of health education to help in improving the health status of these patients. B- Recommendation for further research  A similar study can be replicated at different settings and on a larger probability sample to allow for greater generalization of the findings.
