Abstract

Laryngeal carcinoma growth and invasion patterns are determined by the site of origin and anatomical barriers in the vicinity of the tumour. The presence of local barriers is most pronounced in supraglottic tumours, which tend to spread horizontally within the supraglottic region and rarely extend to the glottic region.
The embryological hypothesis of confinement of supraglottic cancer to the supraglottic regions is not rational due to the lack of a definite histological or anatomical demarcation. Within the supraglottic larynx, two sub-regions can be distinguished: the epilarynx and the lower supraglottis. Lower supraglottic tumours tend to spread anteriorly into the preepiglottic space and remain confined within the supraglottic region, while marginal tumours spread inferiorly to involve the glottic region and pyriform sinus and spread upward towards the tongue base. Cancer arising from the infra-hyoid epiglottis invades anteriorly into the preepiglottic space through the fenestrae of the epiglottic cartilage and remains restrained within this space.
