Abstract

The study showed the following results:  Synovitis was detected in 33 subjects (44%), with 20 (51.3%) of all UC patients, 6 (54.5%) of all CD patients, and 7 (28%) in the control group.  All synovitis cases were without Doppler signals, except for two patients in the UC group who had grade 1 Doppler signals.  The most affected joints with synovitis were the left 2nd metacarpophalangeal (MCP) joint, followed by the left wrist, right wrist, right 3rd MCP joint, and left 2nd and 3rd proximal interphalangeal (PIP) joints.  The study diagnosed two patients with tenosynovitis, two patients with bone erosion, and two patients with osteophytes.  A significant positive correlation was revealed between disease severity (activity) and fecal calprotectin levels. Additionally, a high degree of agreement was demonstrated between sonographic diagnosis of synovitis and the presence of clinical musculoskeletal manifestations, indicating a positive correlation between synovitis diagnosed by MSUS and musculoskeletal manifestations. Furthermore, an association was found between patients receiving biological therapy and normal MSUS results, reflecting the role of biological therapy in treating peripheral arthropathy in IBD patients. The study also showed a relation between synovitis and high readings of ESR, CRP, and fecal calprotectin, with significant differences between the group of patients who had synovitis and those who didn’t.
