Abstract

there were 660 patients with colorectal cancer from Minia governorate during the years 2015-2019, representing an incidence of 4.89% of all cancers and ASR 11.9. 53% were females with Male to Female ratio of 1:1.12, mean age of diagnosis ± SD = 49.87 ± 15.18, 48% of the population were less than 50 years old, the cities of Minia, Abu Qurqas and samalout had the greatest number of cases (24.39%, 15.95%,15.34% respectively), 11.97% were heavy smokers, 2.42% had a positive family history of colorectal cancer. HTN, HCV infection, DM were the most prevalent associated chronic diseases (13.03% ,10.45%, 8.64% respectively). The most frequent presenting symptoms were abdominal pain (80.15%), constipation (53.48%) and bleeding per rectum (50.61%). mean duration of symptoms before diagnosis ± SD =5.62 ± 7.34 months. An increase in the duration of symptoms before diagnosis was correlated with increase in the pathological grade (p <0.001). Ascending colon, rectum, sigmoid colon cancers represented (17.42%, 16.82%, 15.45% respectively). (5.30%) had multiple colon lesions, (44.55%) had underlying adenoma with high grade dysplasia. (37.7%) presented with acute intestinal obstruction necessitating urgent surgery and urgent colostomy Adenocarcinoma, mucinous adenocarcinoma, signet ring carcinoma represented (71.97 %), (14.24%), (5.15%) respectively. Moderately differentiated adenocarcinoma represents 82.12%, LVI was present in 11.97%, PNI was present in 5.61%. (57.58%), (19.09%), (10.45%), (29.24%) had stage III, IV, I, II CRC respectively. 78.57%, 13.49%, 20.63% had simultaneous pathologically proven peritoneal, ovarian, liver metastasis. 98 % of the reported kras gene testing was wild type, The 3-year survival rate of CRC was 57.9%. survival rates were 52.53% and 62.06% in males and females respectively. The Survival duration ranged from 4 to 59 months with mean ± SD = 28.55 ± 16.99, 3-year survival rate was 59.36 %, 19.1%, 78.57%, 74.32 % in stages III, IV, I, II respectively
