Abstract

The finest placement of immediate implants within molar sockets frequently involves drilling to be within the inter-radicular septum. However, this can be difficult when employing conventional preparation methods. Nevertheless, the immediate implant insertion in molar cavities is a viable technique that yields favourable outcomes in terms of both success and survival ratios.
Aim of the study: To evaluate immediate implant placement in mandibular molar inter-radicular septum using osseodensification technique.
Materials and methods: This study was a single-arm clinical trial. Ten patients with ten badly destructed lower molars were selected to match a list of inclusion and exclusion criteria. The participants underwent atraumatic tooth extraction, then the inter-radicular septum was densified using densah burs and immediate implants were installed, followed by bone graft placement in the gap between the implant and the socket walls, and healing abutment was placed. Interrupted sutures were then placed around the healing abutment.
Assessment included measurements of implant stability clinically using the osstell device immediately after implant placement and after three months post-operatively. Bone density, alveolar bone width and marginal bone height were assessed radiographically over three months.
Results: Secondary stability three months postoperatively was significantly increased compared to primary stability measured immediately after implant insertion. Also, the bone density was significantly increased immediately postoperatively and three months postoperatively compared with preoperatively. A significant increase in bone width immediately postoperatively compared with the bone width preoperatively. The marginal bone height was significantly decreased three months postoperatively compared with immediately postoperative.
Conclusion: The immediate implant placement in mandibular molar inter-radicular septum is a successful treatment for the replacement of badly destructed mandibular molars, and densah burs produce an increase in primary and secondary stability, bone density and alveolar bone width.
