Abstract

The study aimed to three dimensionally evaluate maxillary anterior teeth intrusion for treatment of anterior deep bite combined with Gummy Smile using TMA arch wire supported on two posterior miniscrews versus direct intrusion on two anterior miniscrews by using elastomeric chain.
A clinical double- armed study was conducted on 20 patients who were subdivided into two groups the first group received two miniscrews that were placed between upper second premolars and upper first molars and the second group received two miniscrews that were placed between upper canines and upper lateral incisors. Cone beam computerized tomography (CBCT) were taken before and after intrusion to assess the effect of the used intrusion protocols for each patient to compare intrusion amount and detect the presence of resorption in the roots.
The results showed that there were insignificant differences regarding the intrusion amount and resorption of roots between both groups as both groups used intrusion mechanics that were similarly near to center of resistance of anterior segment, but the amount of flaring was slightly lower in group A than group B.
Based on the circumstances of this study, it is apparent that the following conclusions can be drawn:
1. Flaring was slightly lower in two posterior miniscrew group than two anterior miniscrew group because of cinching back the TMA archwire in the two posterior miniscrew group however, this was not applicable in the two anterior miniscrew group.
2. The occurrence of root resorption was minimally observed as a result of incisor intrusion.
3. The utilization of miniscrews and a segmental archwire to intrude the maxillary incisors proved advantageous in reducing incisor protrusion and achieving true intrusion in patients with deepbite.
4. In over bite reduction there was statistically insignificant distinction was seen between the two groups, as the intrusion mechanics in both groups were found to be similarly located near to the center of resistance of the anterior section.
5. Gummy smile was significantly improved after maxillary anterior teeth intrusion.
