Abstract

Diabesity is the 21th century pandemic. Obesity and type 2 diabetes mellitus both represent significant public health challenges. Despite the great advance in diagnosis and treatment of both diseases but still lacking good control of diabetes mellitus and increased incidence of diabetic complications. Over the past 2 decades, clinicians have found that obese type 2 diabetes mellitus patients underwent bariatric surgery for weight loss show improved glycemic control. Hence the emerging importance of interventional diabetology in remission of type 2 diabetes mellitus patients.
The study was carried out in Assiut (plastic surgery department in Assiut university hospital &private clinics).
After informed consent, a prospective interventional study started by 90 patients were assigned into two groups, group A including 45 patients with uncontrolled type 2 diabetes mellitus underwent Roux en y gastric bypass and group B including 45 patients with uncontrolled type 2 diabetes mellitus continued their strict medical treatment and then reevaluated as regard’s their patient characteristics, clinical data, comorbidity and laboratory findings.
We described the result of demographic data and were compared between the 2 groups. It was found that no statistically significant difference in ages in years, gender, residence and work.
In the current study, the mean age at presentation was 49.22+-5.63 years in group A and 47.82+-5.4 years in group B
Regarding as gender, 31 patients (68.9%) in group A and 30 patients (66.7%) in group B were female individuals.
The results of the present study showing that, significant reduction of BMI in bypass surgery patients, significant reduction on the level of lipid profile of bypass patients (triglyceride, LDL, HDL) and on the level of systemic blood pressure (systolic and diastolic).
There were a significant reduction of HBA1C, plasma glucose level, and insulin levels in bypass surgery patients compared with baseline levels and considerable increase in GLP1 levels post surgery.
