Abstract

Hysterectomy is an operation to remove the uterus, cervix, and sometimes ovaries and fallopian tubes. Today hysterectomy is the common surgery performed in the gynecological area, which aids in the decrease of many gynecological problems.Uterine fibroid; uterine prolapse; dysfunctional uterine hemorrhage; and uterine,ovarian, and cervical cancer are all common reasons for hysterectomy. Women who go through hysterectomy face many physical, psychological, emotional, social, and sexual problems both prior to and following the operation. The major factors contributing to these problems are shortage of appropriate information, lack of support and counseling, fears and apprehensions born out of incorrect information.So, it is important to employ qualified psychiatric nurses in order to interact with the hysterectomy women and their families. The aim of such interaction should be ensuring that the woman copes better with the hysterectomy and post-hysterectomy consequence by decreasing the problems encountered by the hysterectomy women (Hassan, et al., 2022).The purpose of the study was to:Evaluate the effect of psychological counseling on posttraumatic stress disorder and marital satisfaction among women with hysterectomy.Research hypothesis 1- The women who receive psychological counseling will have lower mean scores of posttraumatic stress disorder than women who don’t receive psychological counseling.2- The women who receive psychological counseling will have higher score of marital satisfaction than women who don’t receive psychological counseling.Design: A Quazi - experimental research design two groups (study and control group pre/ posttest) was used to achieve the purpose of the study.Setting: This study was conducted at the Obstetrics& gynecology outpatient clinic and surgical outpatient clinic of Menoufia University Hospital, Shebin Elkom,Menoufia Governorate, Egypt.A purposive sample of 100 women who undergone hysterectomy and attending Obstetrics & gynecology or Surgical outpatient clinic of Menoufia University Hospital and who had the following inclusion and exclusion criteria.Inclusion criteria:- Married women of age ranged from 18- 45.- Women had any type of hysterectomy 3 months later.- Had post-traumatic stress disorder.Exclusion criteria:- Widowed and divorced women to be able to assess marital satisfaction.- Having past history of any psychiatric disorders as schizophrenia, bipolar disorder, psychosis that could affect psychological condition.- Having other life-threatening illness or medical conditions associated with an increased risk of PTSD , mastectomy, heart attack, intensive-care unit hospitalization and stroke or have loved ones of those who experience life-threatening illnesses because also is risk for developing PTSD.The following instruments were used to fulfill the aims of the study:Instrument one: A structured interviewing questionnaire: This questionnaire was developed by the researcher to assess social and clinical characteristics of the subjects.Instrument two: The PTSD Checklist for DSM-5 (PCL-5):This scale was originally developed by Weathers, et al., (2013) to measure the presence and severity of PTSD symptoms.Instrument three: Marital satisfaction scale:It was an Arabic scale developed by Al-Talaa & al-Sharif, (2011) to measure marital satisfaction.Instrument four: Patient Health Questionnaire Depression Scale(PHQ-9): Developed by Kroenke, et al., (2001) to assess depressive symptoms.The main findings of the study revealed the following:• The study and control groups were matched as regards to all items of socio-demographic characteristics except for residence where there was a significant higher percent of individuals of study group were from urban area.• There was no significant difference between the study group and control group regarding clinical characteristics’ data.• There was no significant difference between the study group and control group regarding mean score of PTSD pre-intervention while there was a significant reduction of PTSD mean score among the study group than the control group post intervention.• There was no significant difference between the study group and control group regarding mean score of depressive symptoms pre-intervention, while there was a significant reduction of the mean score of depressive symptoms in the study group than the control group post-intervention.• There was no significant difference between the study group and control group regarding of marital satisfaction subscales and total mean score pre-intervention, while there was a statistically significant improvement in the mean scores of affective satisfaction, sexual satisfaction, family problems subscales and total marital satisfaction score among the study group than control group post-intervention.• There was a statistically significant reduction in PTSD mean score among the study group post-intervention than pre-intervention.• There was a statistically significant improvement in the total mean score of marital satisfaction among the study group post-intervention than pre-intervention.• There was a statistically significant improvement in the mean score of all subscales of marital satisfaction (except economic satisfaction and time spending) among the study group post-intervention than pre-intervention.• There was a statistically significant reduction in depressive symptoms mean score among the study group post-intervention than pre-intervention.• There was a statistically significant negative correlation between total mean score of PTSD and marital satisfaction among the study group pre and post intervention. On the other hand, there was a statistically significant positive correlation between total mean score of PTSD and depressive symptoms among the study group pre and post intervention. Also, there was a statistically significant negative correlation between total mean score of marital satisfaction and depressive symptoms.• Higher levels of PTSD among the study group pre and post intervention were significantly associated with younger age and shorter duration of marriage.• Higher levels of PTSD among the study group pre and post-intervention were significantly associated with bleeding during or after labor followed by uterine prolapse and cancer as reasons for hysterectomy.• Higher levels of marital satisfaction among the study group pre and post intervention were significantly associated with older aged wife and with longer duration of marriage.• There was no significant relationship between clinical characteristics and marital satisfaction among the study group pre and post-intervention.• Severe depressive symptoms among the study group pre-intervention were significantly associated with bachelor’s degree educational level. And post intervention it was significantly associated with younger age wife and with shorter duration of marriage.• There was no significant relationship between clinical characteristics and depressive symptoms among the study group pre and post intervention.Conclusion:Based on the result of this study: It was concluded that:The psychological counseling has a positive effect on reducing posttraumatic stress disorder symptoms and depressive symptoms and improving marital satisfaction among women with hysterectomy.Based on the findings of the present study, it could be recommended that:1. Incorporating psychological counseling into current treatment guidelines as a psychosocial option for women with hysterectomy.2. Increasing public awareness about psycho-social complication after hysterectomy as PTSD, depressive symptoms and marital dissatisfaction.In further research:1. Future research is recommended for evaluating the effects of psychological counseling on the women coping with hysterectomy, quality of life, and psychological well-being.2. Replication of the study using a larger sample in different correctional settings for further confirmation and generalizability of the results.
