Abstract

Hypospadias is one of the commonest congenital anomalies of the male genital system. Hypospadias associated with functional and psychological problems for patients and their parents. Many techniques have been described for repairing hypospadias, but none was considered the standard method. The surgical goals of hypospadias repair are full straightening of the penis, formation of a hairless urethra of uniform caliber and adequate size, positioning of the meatus at the tip of the glans and normal penile appearance with minimum complications. Hypospadias that is combined with severe chordae is a serious type of the condition. It is difficult to correct the malformation, and there are still many postoperative complications after surgery. After many years of development, both the operative skills for treating hypospadias and the cure rate have greatly improved, but there is no perfect surgical method to avoid postoperative complication. Controversy still exists with regard to the choice of single-stage or two-stage surgery. We have found that as long as the surgical technique was mature, the postoperative complications of single-stage surgery would not exceed that of two stages Surgery; single-stage surgery can reduce the number of surgeries to one and reduce the burden on the patients and the pain of a second operation.
Conclusion
Hypospadias with marked chordae is a serious condition, and the singlestage correction is an effective surgical procedure for treating this condition, with short operative time and less blood loss without need for second stage. The postoperative complications of single-stage surgery would not exceed that of two stage surgery. Two-step technique has a relevant disadvantage, namely that of the second operation with its additional complications as Well as the negative psychosocial effects on the patient and His family. During the operation, more attention should be given to protecting the blood supply of the vascular pedicle in the foreskin, completely correcting chordae. The use of postoperative care and disinfection should also be strengthened.
