Abstract

Conventional hemorrhoidectomy is the standard surgical treatment for management of grade III and IV HD, however, it is associated with annoying postoperative complications. We aim to compare diathermy hemorrhoidectomy, versus LigaSureTM hemorrhoidectomy, versus Laser hemorrhoidoplasty.
This study was conducted in the Department of surgery, Colorectal unit, at Alexandria Main University Hospital and designed as a prospective comparative study. 45 patients with grade III and IV HD were enrolled in our study, 15 patients treated with DH (group A), 15 patients with LH (group B), and 15 patients with LHP (group C). Assessment of postoperative pain, bleeding, operative time, and time to return to work were recorded and analyzed.
The postoperative pain according to VAS scores from the evening of the procedure till the fourth week postoperatively were significantly higher in DH group patients compared to LH and LHP group, however, no significant statistical difference was found between LH and LHP group. There was significant statistical difference between each group for operative time, intra-operative bleeding, and time to return to work with advantage for LHP ( 11 to 18 min, 12 to 24 ml, and 6 to 10 days respectively ).
According to the results obtained from our study, LHP and LH were associated with less post-operative pain in comparison with DH. LHP was associated with less intra-operative time and bleeding, and time taken to return to work, when compared with LH and DH.
