Abstract

The incidence and prevalence of diabetes mellitus has risen dramatically. Diabetic kidney disease (DKD) is a frequent long-term complication of diabetes and considers the leading cause of chronic kidney disease (CKD) and end-stage kidney disease (ESKD) .DN is defined by the presence of elevated urinary albumin excretion (albumin-to-creatine ratio [ACR] ≥ 30 mg/g) in a person with diabetes .One of the novel biomarkers for the detection of Diabetic nephropathy is FABP1, which is investigated as a marker to detect patient with diabetic nephropathy in T2DM.The Aim of This study is to evaluate clinical usefulness of serum FABP1 level in type 1 DM as novel biomarker of Diabetic Nephropathy in children with T1DM.In present study, We studied 90 children, 60 children with T1DM from Outpatient Clinic of pediatric Diabetes and 30 age, sex matched children as healthy control from general pediatric clinic. The diabetic Children were divided into 2 groups: The first group included patients who are T1DM with diabetic nephropathy. The second group included patients who are T1DM without diabetic Nephropathy. Results:WBCs count is statistically significantly higher in healthy control vs. the two diabetic groups.Serum total cholesterol, and LDL-C are statistically significantly higher in the two diabetic groups vs. the healthy control group.serum cholesterol at a cutoff value > 172 mg/dl is a statistically significant discriminator of DKD from non-DKD .Serum LDL-cholesterol at a cutoff value > 118 mg/dl is a statistically significant discriminator of DKD from non-DKD.FBG is statistically significantly higher in DKD group vs. Non-DKD group.FABP1 is a statistically significantly higher in diabetic nephropathy group vs. diabetic non nephropathy group and control group but not between diabetic non nephropathy group vs. control group.FABP1 at cutoff values of >189.3, and >225.2 are statistically significant discriminator of DKD group vs. DM without DKD and control group, respectively. There is positive correlation between FABP1 vs. ACR, total cholesterol, and LDL-C.Conclusion from the present study, we concluded serum FABP1 may be used as a marker of diabetic nephropathy in patients with T1DM, which will allow for early detection and management of diabetic nephropathy.
