Abstract

This study is conducted to measure the rate of toxoplasma infection in infertile males compared to fertile males as a control serology also to detect association between toxoplasma infection and total testosterone to estradiol ratio in thesera of infertile males.
This study was carried out on two hundreds patient, 100 males with idiopathic infertility and 100 matched fertile controls recruited from Andrology Clinic of Beni-Suef University Hospital.
With a mean age of 33.85 7.50 SD in the cases group and 38.41 9.56 SD in the control group, the study population’s prevalence of diabetes mellitus and hypertension was statistically significant (n = 200).
Anti-Toxoplasma IgG and IgM levels were checked in all serum samples.and it was found that 59 (29,5%) of the total samples (200%) tested positive. Anti-Toxoplasma IgM was positive in (8.0%) of cases compared to (5.0%) in control groups, but it was statistically non-significant with a P value of (0.390) since it was present in only 34.0% of cases compared to 12.0% in the control group.
Finding of anti-Toxoplasma IgG seropositive cases in this regression model was considered a high statistically significant predictive factor for infertility and control groups (P value <0.001 and OR=3.778), while anti-Toxoplasma IgM seropositive cases was non significant but had a strong predictive factor for both infertility and control groups (P value= 0.390 and OR=1.652).
Samples were tested serologically for determination of total Testosterone and Estradiol levels (T:E ratio) and recorded increase of the mean and SD of their levels.
Regards T:E ratio it was disturbed in (38.2%) of Toxoplasma IgG antibodies seropositive cases in infertility group and (41.7%) of the control group statistically insignificant, too. as P value =1 while T:E ratio was disturbed in (15.8%) of anti-Toxoplasma IgMseropositive cases and (84.2%) of the seronegative cases and it was statistically significant as P value =0.051.
It was recorded recorded that Mean and SD of sperm parameters (sperm concentration,abnormal forms and progressive motility) of anti-Toxoplasma IgG seropositive cases decreased in comparison to control group .
The outcomes of the semen analysis revealed a significant difference (P0.01 and P =0.048) in anti-Toxoplasma IgG and IgM seropositive cases regards Oligospermia and Oligoasthenospermia in infertile group respectively.
The frequency of anti-Toxoplasma seropositivity within the infertile group reported a higher frequency in the secondary infertility group (82.4%) than that of the primary infertility group (17.6%).
Our findings showed that (79.4%) ofToxoplasma IgG antibodies seropositive cases were from rural areas and (20.6%) were from urban areas in the infertility group while in control group it showed that (91.7%) of anti-Toxoplasma IgG positive patients were from rural areas and (8.3%) werefrom urban areas but statistically nonsignificant.
The obtained results recorded that (52.9%) and (41.7%) of anti-Toxoplasma IgG seropositive cases in the infertility and control groups were farmers respectively.
The obtained data recorded that (73.5%) of anti-Toxoplasma IgG seropositive cases were of blood group O in infertility and (66.7%) in control group It was statistically nonsignificant as P value = 0.866 .
Regards smoking’s impact on the infection with anti-Toxoplasma seropositive cases in infertile patients it was recorded that (97.1%) of anti-Toxoplasma IgG seropositive cases were smokers in the infertile group and they were 75.0% in the control group.
Chronic diseases results revealed statistically significant association between DM and anti-Toxoplasma seropositive patients as recorded that (97.1%) of anti-Toxoplasma IgG seropositive cases were diabetic in infertility group.
