Abstract

In our country as well as the various parts of the globe, urinary tract stone disease plays a significant role in routine urological care. Although PCNL is a difficult surgery with significant risks, it can be carried out successfully to increase the percentage of patients who are stone-free.
Percutaneous nephrolithotomy in prone position is more favourable by urologist recently supine pcnl has take place due to its promising results with less complications and its ease to anathesia .
The aim of this study is to assess the surgical outcomes of supine pcnl .
Our study is prospective clinical study at Nephrology and Urology Minia University Hospital, involving percutaneous renal surgery using pneumatic lithotripsy in patients with renal stones .
This study was conducted on 53 patients (31male & 22 female) , age range 16 to 65 years old.
In our study the exposure to radiation was ranged from 3.00 – 20.00 min with mean of(11.57 ± 3.85) .
In our study dilatation time ranging from 1.0 to 7.0 with mean (3.85 ± 1.41)
In our study operative time range 30.0 to 100.0 with mean 53.55 ± 14.18 .
In our study the stone free rate was (88.7%).
In our study complications We found extravasationin 1.9% , infection in 5.6% , blood transfusion in 9.5% , icu admission in 1.9 % .

Conclusion
Supine PCNL is a good alternative to standard prone PCNL due to its simple positioning, safety in high-risk patients, good stone free rate, less complications, and ease to the anesthesiologist.
Finally, the following conclusions could be drawn from our study: Supine PCNL is an easy, effective, and time saving procedure compared to prone PCNL for the management of renal stones.
Those with renal abnormalities also benefit greatly from it.
Further research with a bigger sample size and a longer follow-up period is required to corroborate our findings and pinpoint the risk factors for unfavourable outcomes.
