Abstract

The simple objective of any pregnancy is the delivery of a healthy baby to a healthy mother. Adverse maternal and perinatal outcomes have been related to prolonged second stage of labor. Avoiding such adverse outcomes depends on the safe and effective management of this stage Digital transvaginal examination remains the mainstay for clinical evaluation of head descent. Head station is based on the relationship of the leading edge of the fetal head and maternal pelvic landmarks, namely the ischial spine. However, the latter method has proven to be inaccurate and poorly reproducible. The intrapartum transperineal ultrasonography is highly effective in evaluating phenomena related to the passage of the fetus through the birth canal and is a safe tool for assessing the station of the fetal head as well as for predicting the delivery outcomes. Measurements of several parameters, including of the angle of progression (AOP) and head–perineum distance (HPD), have been employed using intrapartum transperineal ultrasonography. AOP and HPD determined by intrapartum ultrasonography were associated with duration of labor, which may aid in predicting the type of delivery in association with clinical parameters. The main results of our study showed that: Age was ranged between 20-40 years with mean value 27.38±4.343 years. BMI was ranged between 26.1-35.0 kg/m2 with mean value 29.80±2.240 kg/m2. Gestational age by US was ranged between 36-40 weeks with mean value 37.91±1.027 weeks. BPD was ranged between 89-102 mm with mean value 95.69±2.481 mm. Birth Weight was ranged between 2750-4210 gm with mean value 3332.68±290.650 gm. FL was ranged between 70- 83 mm with mean value 77.05±2.341. HC:AC was ranged between 0.9-1.14 with mean value 1.00±0.050. BPP was ranged between 8-10 with mean value 8.88±0.992. RI show that only one patient was abnormal. HPD was ranged between 1.0-4.0 cm with mean value 2.87±0.728. Angle of progression was ranged between 125-143° with mean value 134.74±5.032°. Head station was ranged between -2 – +2 with mean value 0.18±1.102. AF status show that 5(7.7%) had meconium. 11(16.9%) had moulding. 11(16.9%) had caput. Mode of delivery showed that 58(89.2%) had vaginal delivery while 7(10.8%) had CS delivery. 11(16.9%) had moulding. 11(16.9%) had caput. Length of 2nd stage was ranged between 40-129 min with mean value 54.07 - 21.4 . Regarding to relation between mode of delivery and HPD & angle of progression , there was positive highly statistically significant correlation between AOP with vaginal delivery patients versus CS delivery patients , while it showed negative highly statistically correlation between HPD with vaginal delivery patients versus CS delivery patients . Correlation between Length of 2nd stage and HPD & angle of progression showed positive highly statistically significant correlation between length of 2nd stage and HPD while there were negative highly statistically significant correlation between length of 2nd stage and angle of progression.
