Abstract

Background: Degenerative Cervical disc disease accompanied by cervical kyphosis is an age-related disease of the cervical spine and represents one of the most common causes of spinal cord dysfunction. Surgical intervention is one of the methods of treatment after failure of all conservative maneuvers. The debate between anterior or posterior approach for pathologies such as cervical spondylotickyphosis have drawn heated debate but are still inconclusive.
Purpose: To compare the anterior and posterior approach used to treat multilevel cervical disc disease with cervical kyphosis, focusing on sagittal alignment, functional and clinical outcome.
Patient Sample: Prospective randomized comparative study of 40 patients with degenerative cervical kyphosis with radiculopathy or myelopathy divided into 2 groups; 20 patients (group I) had anterior cervical interbody discectomy and fusion (ACDF) and the other 20 patients (group II) had posterior laminectomy and lateral mass fixation with an average of 12 months of follow up.
Outcome Measures: Patients were assessed functionally with Visual Analog Score (VAS) for neck pain and upper limb pain, Neck Disability Index (NDI), modified Japanese Orthopedic Association scale (mJOA) and complications. Radiological fusion was confirmed with serial plain X-rays.
Methods: It consists of a prospectively collected consecutive series of 40 patients with degenerative cervical kyphosis who met inclusion criteria between August 2018 and March 2020 were done in Kasr Al Ainy hospital, Cairo University and divided into two groups; The study included 29 males and 11 females.
