Abstract

Radical hysterectomy has an excellent oncological outcome in cases of endometrial and cervical cancer. However, post-operative urodynamic complications have been a concern. The etiology of the urological abnormalities is most likely related to damage to nerve supply of the urinary system. Comparing the rate of different post-operative complications following the nerve-sparing technique and the conventional non-nerve sparing technique shows significant superiority of the nerve-sparing technique in terms of urological safety. Two weeks post-operatively, 97.1% of the non-nerve sparing group had urinary symptoms and 94.3% had confirmed urodynamic abnormalities upon investigations, compared to only 28.6% and 0% in the nerve-sparing group, respectively. Three months post-operatively, the whole nerve-sparing group was urodynamically-normal, while 25.7% of the non-nerve sparing group was still sustaining a urodynamic abnormality (possibly due to nerve cut injuries). Additionally, the nerve-sparing group had faster recovery of normal urinary functions. Previously published studies, both primary and secondary studies, from different parts of the world agree with our findings. This strongly indicates a significantly safer profile for the nerve sparing technique across different populations with different characteristics
