Abstract

Introduction: Cardiovascular diseases are the number one cause of death worldwide. Analysis of the underlying cause specific cardiac arrest and implementation of high quality cardiopulmonary resuscitation (CPR) can develop action plans for improving survival to hospital discharge. Objectives: To identify the cause specific cardiac arrest, to evaluate adherence to CPR guidelines and to investigate different variables affecting return of spontaneous circulation and survival to hospital discharge after in-hospital cardiac arrest. Methods: A prospective observational study which included 2563 patients admitted to cardiology departments of Kasr Alainy hospital, elmanial specialized hospital, and New Kasr Alainy teaching hospital during the time period from May 15th 2014 to February 28th 2015. Out of these patients, 141 patients had in hospital cardiac arrest. Results: The main cause of admission among the total population was coronary artery disease found in 935 patients (36.5%), while the main cause of cardiac arrest patients was congestive heart failure/cardiogenic shock found in 32 patients (22.7%). CPR was attempted in 132 patients (93.6%), while it was not attempted in 9 patients (2 patients found dead and 7 patients were considered futile). The initial rhythm was shockable in 20 patients (14.4%) and non shockable in 119 patients (85.6%). The optimum cut off value for CPR duration in predicting return of spontaneous circulation (ROSC) was noted to be 22.5 minutes with a sensitivity of 75.8% and a specificity of 66.7%
