Abstract

Background: fracture of the distal tibia with unfavorable skin condition present a management dilemma because of subcutaneous position of distal tibia and scanty blood supply of this region. Supracutaneous fixation reduces more soft tissue injury with no interruption of blood supply.Patient and methods: the current study was done on 30 distal tibial fractures. The mean age was 39.6 ±13.6 years. Mode of trauma was road traffic accident in eight (27%) patients, fall from hight in twelve (40%) patients, direct trauma by heavy objects in four (13%) patients and fall on the ground in six patients (20%). We managed all patients by using supracutaneous plate fixation. The average operative time was 99 ±15.6 minute. We fixed the fractures in a closed manner. Closed reduction was done under fluoroscopy, appropriate plate length was selected then fracture was fixed.Results: in the current study according to subjective and objective criteria of Ovedia there were ten cases (33.3%) of excellent results, sixteen cases (53.3%) of good results, two cases (6.7%) of fair results, and two cases (6.7%) of poor results in the current series.Conclusion: supracutaneous plate fixation technique yields good to excellent clinical outcomes with accepted functional outcome for management of the distal tibia fractures with unfavorable skin condition
