Abstract Prostate cancer (PCa) is the second most common cancers in men worldwide. Its incidence can be influenced by several risk factors including genetic susceptibility. Therefore the search for certain gene expression (ERG) and its rearrangement could give us clue to proper identification of PCa. To determine the frequency of ERG expression in PCa of Egyptian patients and in HGPIN juxtaposed to PCa. Also to assess the concordance between ERG immunoexpression and ERG-TMPRSS2 rearrangement in PCa. This study was performed on 85 cases of PCa, showing 30 cases with concurrent high grade prostatic intraepithelial neoplasia (HGPIN) and 30 cases of prostatic hyperplasia. All were immunohistochemistry stained using ERG monoclonal rabbit antihuman was used (clone: EP111). TMPRSS2 status, using fluorescence in situ hybridization (FISH) analysis was performed in 38 biopsies of PCa cases to detect ERG-TMPRSS2 rearrangement using the FISH ZytoLight TriCheck Probe (SPEC ERG/TMPRSS2). ERG expression was found in 26% of PCa cases, 20% of HGPIN with co-existing PCa and 6.7% of benign prostatic tissue samples. IHC revealed a concordance, sensitivity and specificity for ERG rearrangement of 95.5%, 100% and 94.4%, respectively. Our findings emphasize that ERG immunohistochemical expression is specific for prostatic neoplasia.
