Abstract

Aim of the work: To compare the diagnostic accuracy of diffusion-weighted MRI (DW-MRI) and single-voxel MR Spectroscopy (SV-MRS) to conventional dynamic contrast-enhanced MRI (DCE-MRI) in the differentiation of benign from malignant breast lesions. Patients and methods: The study was performed using 1.5 T MRI system including 35 female patients with 47 detected breast lesions. All lesions were analyzed for characteristics of morphology, contrast enhancement kinetics curve, qualitative and quantitative analysis of DWI and ADC values in addition to tCho peak heights measurement in SV-MRS. The results of each MRI modality were correlated to the histopathological results. Results: Thirty five female patients with 47 lesions (pathologically proven 32/47 malignant lesions and 15/47 benign lesions) were analyzed. The sensitivity and specificity were 87.5% and 93.3% respectively using DCE-MRI morphological findings, 96.9% and 73.3%, respectively, using DCE-MRI kinetic curve pattern, 96.9% and 66.7%, respectively, using DWI and finally 74.2% and 71.4%, respectively, using MRS. The suggested ADC cut off value was 1.063 {u00D7} 10{u2212}3 mm2/s, so ADC values equal to or below this value were considered restricted diffusion. The suggested cut off value of tCho peak was 0.36 mmol/l, so tCho peaks equal to or above this value were considered malignant
