Abstract

This study investigated memory change in patients diagnosed with MDD according to DSM-5 by using valid and reliable tests which are:-
- Mini Mental State Examination, (MMSE)
- Montreal Cognitive Assessment, (MOCA)
- Hamiliton Rating Scale for depression, (HAM-D)
The current study was interventional study, a registered clinical trial (NCT03915821) conducted at psychiatry Unit, Assiut University hospitals, Assiut University. The study was conducted from the 1st of October 2020 till end of June 2021.
After applying inclusion and exclusion criteria, forty participants were enrolled in the study recruited from Psychiatry Unit, in Assiut University Hospital, Assiut, Egypt, in patient who admitted to the department, the patients classified into group A comprised that treated with Unipolar ECT, and group B comprised that treated Bipolar ECT.
Firstly all patients have been examined with history taking and full general & neurological examination to exclude any neurological disease, with history taking and full psychiatric examination to exclude other psychiatric disorders, based on DSM-5.
Unilateral group of patients had their treatment by right unilateral placement electrode with 44.6 j, 253.4 mc,7.920 sec duration , and bilateral group of patients had their treatment by bitemporal electrode placement with 44.6 j,417 mc, 7,920 sec duration , and both group subjected to the three tests at date of admission and after finishing treatment at day of discharge and at follow up after three months.
Our major findings could be summarized into the following:
In demographic data of the studied patient groups showed that there were no statistically significance differences between the groups of two patients in all demographic data, however unilateral group tend to be younger in age.
On the study of Montreal Cognitive Assessment (MOCA) Scale in the studied patients; group I (Unilateral stimulation), it was 14.06 ± 5.74, 20.94 ± 3.56, 23.65 ± 3.74, on admission, at discharge and after 3months follow up respectively. group II (Bilateral stimulation), it 15.39 ± 4.89, 20.17 ± 4.70, 21.35 ± 4.49, on admission, at discharge and after 3months follow up respectively.
In Memory subscale in the studied patients groups according to Montreal Cognitive Assessment showed that On the study of memory subscale of MOCA in the patients; group I (Unilateral stimulation), it was 2.24 ± 1.15, 4.35 ± 0.61, 4.76 ± 0.44, on admission, at discharge and after 3months follow up respectively. group II (Bilateral stimulation), it was 2.26 ± 1.32, 4.13 ± 0.63, 4.83 ± 0.39, on admission, at discharge and after 3months follow up respectively.
On the study of Hamilton depression scale (HAM-D) in the studied patients.in the studied patients; group I (Unilateral stimulation), it was 32.06 ± 4.94, 3.53 ± 1.12, 2.00 ± 1.00, on admission, at discharge and after 3months follow up respectively. group II (Bilateral stimulation), it 30.65 ± 5.58, 3.83 ± 1.27, 3.04 ± 1.33, on admission, at discharge and after 3months follow up respectively.
On the study of Mini-Mental State Examination in the studied patients; group I (Unilateral stimulation), it was 18.71 ± 7.02, 27.47 ± 2.35, 28.06 ± 1.64, on admission, at discharge and after 3months follow up respectively. group II (Bilateral stimulation), it 21.22 ± 5.01, 26.09 ± 2.97, 26.65 ± 2.31, on admission, at discharge and after 3months follow up respectively.
On the study of Memory subscale according to Mini-Mental State Examination (MMSE) in the studied patients; group I (Unilateral stimulation), it was 1.65 ± 1.06, 2.82 ± 0.39, 3.00 ± 0.00, on admission, at discharge and after 3months follow up respectively. group II (Bilateral stimulation), it 1.61 ± 0.94, 2.91 ± 0.29, 3.00 ± 0.00, on admission, at discharge and after 3months follow up respectively.
On the study of Correlation between Hamilton Depression Scale and memory sub-scales of Montreal Cognitive Assessment and Mini-mental State Examination; the P-value of memory subscale of MOCA was 0.050, 0.015, and the R-value was -0.311,-0.382 respectively.
