Abstract

Introduction:Many techniques are available for correcting a wide alar base and flared alar lobules, including sill excision alone, alar excision alone, using cinching sutures to pull the ala together, the V-Y advancement technique and the alar flap advancement technique. Joseph and Milstein described narrowing of the alar base by using internal excisions from the nostril base and vestibular floor. This concept was later modified and popularized by many authors, mainly to avoid the external scar that follows the classic Wier excision. Objective: To evaluate the role of Internal alar resection technique in correcting alar flare and narrowing the nasal base with consequent improvement in nasal harmony in the whole face. Methods: A prospective study, by analysis of surgical outcomes and photos of 25 patients presenting with a wide nasal base and alar flaring. The surgical procedure internal alar resection was done. This study was conducted at Kasr Al-Aini hospital, department of plastic and reconstructive surgery, between October 2017 till March 2018. Results: An improvement by decreasing the inter-alar distance was achieved in 100% of cases. Twenty five patients were having alar flare before the surgery, and after the surgery; 18 patients (72%) are completely corrected, and only 7 patients (28 %) are still having alar flaring BUT in a lesser degree. Conclusion: Internal alar resection effectively reduced the alar flare, and improve the nostril shape and width, this technique was also useful for correcting the symmetry of nostril shape, hide the scars internally in the nose, avoid the injury of lateral nasal artery and preserve the natural curvature of alar rim. Thus, it can be a good option for correction of alar flare and narrowing alar base
