Abstract

Lesions in the pulley system of the long head biceps tendon (LHBT) are a distinct entity and a frequent cause of anterior shoulder pain.
The aim of the study was to evaluate the incidence of pulley lesion associated with cases underwent to arthroscopic repair of rotator cuff tears.
Our study included 60 patients underwent arthroscopic rotator cuff repair at Al-Hadra university hospital. The age of patients ranged from 40 to 65 years. Forty three patients (71.7%) were females and seventeen patients (28.3%) were males. Injury of biceps pulley was found in 83% (29 of 35 cases) of degenerative cuff tear and 88% (22 of 25 cases) of traumatic cuff tear.
The overall incidence of pulley lesion was 85%. Some predictive risk factors were studied, some of which can be correlated as predictive risk factors of biceps pulley lesion as age of the patient where the older the patient with cuff tear, the more likely pulley lesion will occur and the chance for surgical management (tenotomy or tenodesis) will increase.
Some risk factors weren’t significantly associated with pulley lesion as gender of the patient, the mode of cuff injury.
There were no specific MRI signs of biceps pulley lesion but there are signs that may indicate presence of lesion with arthroscopic assessment as subluxation or dislocation of LHBT.
Arthroscopic diagnosis of pulley lesions remains the gold standard diagnostic method
