Abstract

Aim of the Study: The aim of this study was to evaluate the effect of. z-plasty in the orbicularis orismuscle while maintaining the curviliner skin incision of Millard on the length of repaired lip Patients and Methods:: In this randomized clinical prospective trial A group of thirty patients with nonsyndromic unilateral cleft lip were randomized into two equal groups. group A: undergone modified Millard’s technique, group B: undergone orbicularis oris muscle z-plasty modified Millard’s technique. 6 months postoperatively the patients were photographed to assess the length of philtral ridge, scar width, and nostril width and to ask parents about their satisfaction. Results: No significant difference in philtrum length, scar width, nostril width or parents satisfaction at 6 months postoperatively between the two groups. Conclusion: longer follow up period may be recommended as in the case of wide clefts showed almost perfect symmetry after 1 year as postulated by Millard. The described technique may be a promising strategy to improve lip appearance, and scarring during primary cleft lip repair
