Abstract

Background: Pediatric sepsis causes morbidity, mortality, and high costs worldwide. Early resuscitation of pediatric septic shock improved outcomes. Sepsis induced myocardial dysfunction (SIMD) is serious complication in pediatric septic shock. ECHO is affected by loading condition and inter-observer variability. Serum cardiac troponin I (c T n I) is sensitive and specific to myocardial injury in SIMD.Objective: To study troponin I as a cardiac biomarker in early detection of cardiac dysfunction in pediatric severe sepsis and septic shock. Methods: 80 children aged from 3 months to 12 years classified into: (a) 40 cases in PICU of Cairo University Children, s Hospital having evidence of severe sepsis or septic shock. Serum c T n I was withdrawn in the first 24 hours after admission and the results were compared to clinical data and ECHO findings (b) 40 controls with no sepsis or heart disease.
